NYSPHSAA is partnering with FinalForms for the 2024-25 Participation Survey. Schools are required to submit the information by Monday, May 12, 2025.

2. Access Your Account – Login as "Staff" (FAQs)

3. Complete the Participation Survey – Full Instructions

4. Join a Live Training for Assistance:
